[NEohioPAL] Help with CD Insert

Can anyone recommend someone to help with creating the insert for a CD we are recording?  I basically know what I want but just need someone to help lay it out and to get it printed.  I would appreciate feedback from the list from people who have had them made and were happy with the work.
