Robin,

When our heavy latin jazz trio, /Melocotón/, licensed Radiohead's 
"Paranoid Android" for our debut CD "Perjuvian", we used RightsFlow's 
LimeLight service - www.songclearance.com 
<http://www.songclearance.com>.  It was really easy. You pay based on 
how many CDs, digital downloads, ringtones, and live streams you 
anticipate.  It's something of an honor system, so you keep track of the 
sales and pay more if you use up what you've licensed.

In our case, we planned for 100 physical discs and 100 downloads of the 
song and it cost a bit over $50.
