The 3rd annual Junk2Funk Benefit will be held on Friday,
April 20, 2012 at Local Girl Gallery, 16106 Detroit Avenue, Lakewood. Our
Chinese Raffle will showcase local artists’ “recycled” works of art .  This could be anything from repurposed
furniture, to funky jewelry and clothes to a visual art piece. 
Would you please help by donating a piece of Art, Gift
Basket or Appetizer for the benefit?
For 25 years, North Coast Health Ministry (NCHM) has been
providing healthcare to uninsured people living on Cleveland's West Side.  As faith-based free clinic, we are a bridge
to better health for almost 3,000 individuals who have no other place to go.  With the support of dozens of volunteer
physicians and nurses, as well as clerical volunteers, NCHM is able to provide
primary health care, specialty referrals, prescription assistance, preventative
and chronic disease management, medical tests, behavioral/mental health
counseling and health education to low-income individuals and families.  Each year, the demand for our services
grows.  During the past two years alone,
patient visits increased by nearly 40%.  
All donors will receive recognition in the event program.
Enclosed you will find a donation form to complete and submit with your
donation. Or please call and we will make arrangements to pick it up. All
donations are tax-deductible to the full extent allowed by law.
